Payment
-Payment is required prior to your appointment.
-Installment plans can be set up if needed
-Further appointments cannot be scheduled until you have paid the fee and/or co-pay prior to services provided.
Additional Fees
-A $30 late or no show fee may be charged if you miss you appointment and do not cancel 24 hours prior
